Sprain of 1st costotransverse joint

Definition

  • Often the testing might indicate the scaleni to be problematic, but the reason for this is because the scaleni is attached to 1st rib which is being pulled when the scaleni is stretched or contracted causing pain by the activity and movements of the scaleni

Indications

  • Pain is present at the base of the neck on the one side
  • Some of the movements of the shoulder and arm provokes the pain

Examination

Neck

  • Active lateral flexion away from painful side - painful
  • Passive lateral flexion away from painful side – painful
  • Resisted lateral flexion towards painful side – painful

Shoulder

  • Active elevation – painful
  • Passive elevation - painful
